5Essentials Parent Survey/ Encuesta 5Essentials

The 5Essentials survey is a survey taken by students, parents, and teachers and serves as another tool to improve schools.  Fourth-twelfth grade students throughout Illinois will be taking this survey.  The fourth and fifth grade students at Field will be taking this survey at school.  Results from this survey will be used as part of the State School Report Card.  The purpose of this anonymous survey is to ensure that we as a school continue to improve to meet our high standards.  We encourage all parents to take this survey.  The link for this survey is https://survey.5essentials.org/Illinois/.  This link can also be found on our school website.  The window to take this survey ends on February 15, 2019.  Thank you in advance for your input!

La encuesta 5Essentials es una encuesta realizada por estudiantes, padres y maestros y sirve como otra herramienta para mejorar las escuelas. Los estudiantes de cuarto a doceavo grado de todo Illinois tomarán esta encuesta. Los estudiantes de cuarto y quinto grado en Field tomarán esta encuesta en la escuela. Los resultados de esta encuesta se utilizarán como parte del Informe Escolar del Estado. El propósito de esta encuesta anónima es garantizar que nosotros, como escuela, sigamos mejorando para cumplir con nuestros altos estándares. Animamos a todos los padres a tomar esta encuesta.
